PTP Coaching Mattapan November 2024

Description

MA LP Clinical Director and EHE Program Manager met with CARES team staff for monthly PTP check in.

Discussion of HIV case management successes and challenges.

Discussion of collaboration with OBAT nursing to support long acting injectables and medication distribution. HIV team currently without RN. NEAETC planning for RN training for injectable PrEP in January.

Team provided details about upcoming World AIDS Day event on 12/6. Internal team providing a presentation for all Mattapan providers as well as a patient panel and HIV education and testing.

Discussion of QI project to increase HIV testing rates by individual medical providers.
